7 Certificates relating to amounts paid or payable by the Authority
For the purposes of section 590 of the Workplace Injury Rehabilitation and Compensation Act 2013, a certificate may include any of the following that relate to amounts paid or payable by the Authority under that Act—
the nature of the amount;
if the amount is or relates to a weekly payment, the period in relation to which the weekly payment was paid or is payable;
the invoice number, if any;
the date on which any invoice was issued;
the date on which any invoice was received;
the date on which the professional service, if any, was provided;
the nature of the professional service provided, if any;
the name of the payee;
the date on which any cheque was drawn;
the drawer name on any cheque;
the drawer's ADI;
the date on which any cheque was issued;
the date on which any payment was made;
the date on which any amount was directly debited;
the name of the claimant, if any;
the name of the provider of the professional service, if any;
the provider number of any provider;
the address of any provider;
the WorkCover claim number, if any;
the name of the worker, if any;
the date of birth of any worker;
the date on which any injury occurred.
In this regulation, provider number, in relation to the provider of a professional service, means—
the identification number, if any, allocated to the practice of the provider for the purposes of the Health Insurance Act 1973 of the Commonwealth; or
if there is no such identification number, the identification number allocated by the Authority to—
the provider; or
the practice of the provider.
