20 Details to be included in notice given by supervising authority to Commissioner (s. 70)
For the purposes of section 70(3) of the Act, the following details are prescribed —
(a)
the name of the reportable offender;
(b)
the relevant event listed in section 70(1) of the Act that has occurred;
(c)
the date on which that event occurred;
(d)
the name, title and signature of the supervising authority who gives the notice;
(e)
the date on which the notice is given.
