254 Recording medicines supplied without prescription in emergencies
A pharmacist who supplies a designated prescription only medicine to a person under section 251 must make a written record of the following information in relation to the supply of the medicine:
the pharmacist’s name;
the name of the prescriber who would ordinarily have prescribed the medicine;
the date the medicine is supplied;
the name and address of the person to whom the medicine is supplied;
the medicine’s approved name and brand name;
the form, strength and quantity of the medicine supplied.
