22 Information for written notice about original contact person giving approved substance to another person—Act, s 67 (6)
The following information is prescribed:
(a)
the name, home address or business address and telephone number of the new contact person;
(b)
the day the original contact person gave the approved substance to the person mentioned in the Act, section 67 (2).
