37.04 Notice of appeal

(1) Unless an Act provides otherwise, an appellant commences an appeal by filing, not later than 28 days after a decision is made, a notice of appeal in the registry at a proper venue for the proceedings.

(2)

A notice of appeal is to:

(a)

be in accordance with Form 37A; and

(b)

state:

(i)

the name and address of the appellant;

(ii)

the name and address of the respondent;

(iii)

the decision in respect of which the appeal is brought;

(iv)

the date on which the decision was made; and

(v)

specifically and concisely, the grounds of appeal.

(3) As soon as practicable after filing a notice of appeal, the appellant must serve a copy on the respondent.
