An internal review application must be—
made within 20 business days after—
if the person has been given an information notice about the original decision to which the application relates—the day the person is given the notice; or
if subparagraph (i) does not apply—the day the person otherwise becomes aware of the original decision; and
made in the approved form; and
accompanied by a statement of the grounds on which the applicant seeks the review of the decision; and
supported by enough information to enable the decision to be reviewed.
However, the reviewer may extend the period for making the application.
