the licence number (or details of another authority) under which the sandalwood was taken;

details of the place from which the sandalwood is to be transported;

the name of the person to whom the sandalwood is to be delivered;

the address of the place to which the sandalwood is to be delivered;

the quantities of both dead and green sandalwood that is to be transported;

the form in which the sandalwood is to be transported;

the date on which the sandalwood is to be transported;

the registration number of the vehicle on which the sandalwood is to be transported and the registration number of a trailer attached for that purpose (if any);

the signature of the driver of the vehicle on which the sandalwood is to be transported;

any other information required by the sandalwood tracking form.
