13 Application for internal review
An application for internal review under section 42(3) of the Act must:
(a)
be in writing; and
(b)
specify the name of the affected person; and
(c)
identify the reviewable decision; and
(d) set out the reasons the affected person is seeking a review of the decision; and
(e) specify an address to which correspondence regarding the review may be sent to the affected person.
