33 Documents to be filed with applications for adoption orders

For the purposes of section 24(1) of the Act, a report by the Secretary or a principal officer concerning the proposed adoption of a child by a prospective adoptive parent is to contain the following information:the circumstances of the proposed adoption; the eligibility and suitability of the prospective adoptive parent on whose behalf the application is made to adopt the child; any wishes expressed by the child or a natural parent of the child at the time when consent to the adoption was given; any arrangements agreed between a natural parent of the child and the prospective adoptive parent for access to the child or for information about the child.

For the purposes of section 24(4) of the Act, the form of a report by a medical practitioner as to the physical and mental condition of a child is to be in accordance with – Form 3, if the child to whom the report relates has not attained the age of 6 months; or Form 4, if the child to whom the report relates has attained the age of 6 months or more.
