14 Form of statements by witnesses to consent
(1)
For the purposes of section 34(1)(b) of the Act, the statements in Form 6 of Schedule 1 are prescribed.
(2)
For the purposes of section 34(3)(c) of the Act, the statements in Form 7 of Schedule 1 are prescribed.
