31 Details of adoptions to be provided to the Secretary by approved agencies
For the purposes of section 85(2)(b) of the Act, the following details are prescribed—
(a)
the former registered names, date and place of birth of the child;
(b)
the names of the child that are given in the adoption order;
(c)
the names, address and date and place of birth of each of the natural parents of the child;
(d)
the names, address and date and place of birth of each of the adoptive parents of the child;
(e)
the name and address of the person who made the report referred to in section 15(1) of the Act.
