13 Application for provider approval by person other than an individual
For the purposes of section 11(c) of the Act, the following information is prescribed for an application for a provider approval made by a person other than an individual under section 10 of the Act—
the applicant's name and any trading or other name used by the applicant;
the applicant's street address and postal address or, if there is more than one address, the street address and postal address of the applicant's principal office;
the name and contact details of the contact person for the purposes of the application;
documentary evidence of the legal status of the applicant and its constitution;

The partnership agreement for a partnership.
a financial declaration regarding the applicant;
for each individual who will be a person with management or control of a children's service to be operated by the applicant, the information set out in regulation 12.
Note
If there is more than one applicant, the prescribed information must be provided in respect of each applicant—see section 10(3)(a) of the Act.
