15 Application for amendment of provider approval
For the purposes of section 22(2)(b) of the Act, the following information is prescribed for an application under section 22 of the Act for an amendment of a provider approval—
(a)
the full name of the approved provider;
(b)
the provider approval number;
(c)
the name and contact details of the contact person for the purposes of the application;
(d)
the details of the amendment applied for;
(e)
sufficient information or documentation to support the application for amendment.
