21 Application by person other than an individual for provider approval on incapacity of approved provider

For the purposes of section 40(3)(b) of the Act, the following information is prescribed for an application for a provider approval made under section 40(2) of the Act by the legal personal representative or guardian of an approved provider, if the applicant is a person other than an individual—

(a)

the applicant's name and any trading or other name used by the applicant;

(b)

the applicant's street address and postal address or, if there is more than one address, the street address and postal address of the applicant's principal office;

(c)

the name and contact details of the contact person for the purposes of the application;

(d)

the current approved provider's full name and provider approval number;

(e)

the proposed duration of the provider approval;

Note

This period cannot exceed 6 months—see section 41(4)(a) of the Act.

(f)

documentary evidence of the legal status of the applicant and its constitution;

Example

The partnership agreement for a partnership.

(g)

a financial declaration regarding the applicant;

(h)

for each individual who will be a person with management or control of a children's service to be operated by the applicant, the information set out in regulation 20.
