37 Person may make final request
(1)
A person may make a final request for access to voluntary assisted dying if the person has made a written declaration.
(2)
A final request must be made to the co-ordinating medical practitioner for the person, by that person personally.
(3)
The person may make the final request verbally or by gestures or other means of communication available to the person.
