the person making the appointment; and
the person being appointed.

the person signing—
is aged 18 years or more; and
is not a witness to the signing of the form; and
is not the person to be appointed as the contact person.
A person who signs a contact person appointment form on behalf of the person making the appointment must do so in that person's presence.
If a person appoints a contact person with the assistance of an interpreter, the interpreter must certify on the contact person appointment form that the interpreter provided a true and correct translation of any material translated.
