41 Final review by co-ordinating medical practitioner on receipt of final request
On receipt of a final request made by a person, the co-ordinating medical practitioner for the person must—
review the following forms completed under this Part—
the first assessment report form;
all consulting assessment report forms;
the written declaration;
the contact person appointment form; and
complete the final review form in respect of the person; and
certify whether the request and assessment process has been completed as required by this Act.
The co-ordinating medical practitioner must give the Board a copy of the completed final review form, accompanied by copies of all forms referred to in subsection (1)(a), within 7 days after completing the final review form.
