12 Appointment of corporation representative

An application under section 245(1) or (2) of the Act must be in writing and include the following details—

(a)

the name of the relevant Council;

(b)

the address of the relevant rateable property;

(c)

the personal details of the person to be appointed;

(d)

the office in the corporation held by the person to be appointed;

(e)

a declaration signed and dated by the person to be appointed that states that the person—

(i)

consents to the appointment; and

(ii)

is not otherwise entitled to be enrolled on the voters' roll for the municipal district in respect of which the appointment is to be made;

(f)

the corporation details of the corporation making the application;

(g)

a declaration signed and dated by a person authorised to sign on behalf of the corporation that states that—

(i)

the person is authorised to make the application on behalf of the corporation; and

(ii)

the corporation is an owner or occupier of the rateable property (as the case may be); and

(iii)

the person named in the application as the prospective appointee is a company secretary or a director of the corporation (as the case may be) and is to be appointed to be the voting representative of the corporation.
