Dog Regulations 2013

In this regulation each of the following terms has the meaning given in the Veterinary Practice Act 2021 section 3 —

For section 38N(2)(a), the information in relation to the health status of the dog is —

the date the veterinarian completed the health assessment of the dog; and

the dog’s name; and

the dog’s registration number; and

the unique identification number for the dog’s microchip; and

the dog’s date of birth or age (if known), breed (if known), colour, gender, distinguishing features or marks, sterilisation status and vaccination and parasite treatment history; and

the details of the assessment of the health of the dog carried out by the veterinarian, including whether the dog has any signs of —

being sick or injured; or

an abnormality, defect, deformity or disease that causes, or is likely to cause, a significant impact on its health;

any details of the medical history of the dog (if known); and

any other observations as to the health of the dog.

A health certificate must contain the following information about the veterinarian who completed the health assessment of the dog —

the veterinarian’s name;

a statement as to whether the veterinarian is a WA veterinarian or an interstate veterinarian;

if the veterinarian is a WA veterinarian — the veterinarian’s registration number referred to in the Veterinary Practice Act 2021 section 32(2)(c);

the name of the veterinarian’s veterinary practice business (if any);

the address of the premises at which the health assessment was completed.

A health certificate must contain the following information about the holder of the dog supply approval who obtained the health certificate —

the person’s name and address;

the unique number for the dog supply approval issued under section 38U(1)(b) to the person.
