471 Authorisation condition for opioid dependency treatment licences—Act, s 44 (1) (b) and (2) (b)
A licence-holder’s authorisation under an opioid dependency treatment licence is subject to the following conditions:
the licence-holder must ensure that a person to whom buprenorphine or methadone is administered under the licence signs a written acknowledgment in accordance with subsection (2) that the medicine has been administered to the person;
a purchase order issued by the licence‑holder to obtain buprenorphine or methadone under the licence is a complying purchase order.

For subsection (1) (a), the acknowledgment must include the following:
the approved name or brand name of the medicine administered;
the form, strength and quantity of the medicine administered;
the date the medicine is administered.
