60 Report to be submitted by approved commercial dog breeder
For the purposes of section 58AF(3) of the Act, the following information is prescribed—
the number of breeding and non-breeding dogs on the premises in the reporting period, including the breed and sex of each dog;
the number of litters of puppies born in the reporting period;
for each breed of puppies sold, the breed and number of puppies sold in the reporting period;
the number of relevant fertile female dogs and all other dogs which were retired in the reporting period;
the number of relevant fertile female dogs and all other dogs which were sold in the reporting period;
the number of relevant fertile female dogs and all other dogs which were euthanased, or died of other causes, in the reporting period, and the reason for the euthanasia or death;
the number of puppies returned under the sale guarantee in the reporting period;
the number of new staff that commenced in the reporting period and evidence of their relevant qualifications;
a document that contains a summary of each complaint received in the reporting period including a summary of the responses to those complaints;
details of any disease outbreaks and response to the outbreak in the reporting period;
the specified dog details for each dog kept on the premises during the reporting period;
the details of any changes to any information provided under regulation 57(2) or 58(2);
a copy of an agreement with a veterinary practitioner to provide veterinary services to the business, if the practitioner has changed since giving information under regulation 57(2) or 58(2);
a copy of the current certificate of public liability insurance for the breeding domestic animal business.
In this regulation—
reporting period means the period specified by the Minister.
Note
Provision of a report containing the information prescribed in this regulation is a condition of a commercial dog breeder approval. It is an offence for an approved commercial dog breeder to fail to comply with a condition of a commercial dog breeder approval—see section 58AF(7) of the Act.
