7 Form of report for application for an applicable organisation approval

For the purposes of section 5A(3)(a) of the Act, the following details are prescribed—

(a)

the organisation name and contact details;

(b)

the names and roles of members of the board, committee or other managing body of the organisation, as applicable;

(c)

the breeds of dogs or cats that the organisation represents;

(d)

the number of members of the organisation that conduct breeding;

(e)

the total number of members of the organisation;

(f)

the number of dogs or cats, as applicable, that are registered with the organisation;

(g)

a copy of the organisation's complaints policy and complaints handling procedure;

(h)

a copy of the organisation's membership form that demonstrates how members agree to the conditions of membership and the organisation's code of ethics;

(i)

a copy of the education and training material used by the organisation to inform members that they must comply with the Act, the Prevention of Cruelty to Animals Act 1986 and any other laws relating to the keeping of dogs or cats;

(j)

a copy of the education and training program used by the organisation to ensure staff are trained to enforce the organisation's code of ethics;

(k)

evidence used to demonstrate that each member of the organisation has received and agreed to the conditions of being a member of an applicable organisation;

(l)

a copy of the disciplinary policy and disciplinary hearing procedure;

(m)

a copy of the organisation's procedures and protocols for reporting and handling of incidents including animal cruelty, dog attacks and animal diseases;

(n)

an outline of the organisation's sale guarantee required under the relevant guidelines, health management plan, any current or proposed breeding programs and other breeding-related practices.
