12 Application to change name of incorporated association
For the purposes of section 24(3)(b) of the Act, the prescribed particulars are—
(a)
the current name of the incorporated association; and
(b)
the registration number of the incorporated association; and
(c)
the proposed new name of the incorporated association; and
(d)
the name, address, contact telephone number and email address of the secretary of the incorporated association; and
(e)
the place of the meeting where the special resolution for the change of name was passed.
