For the purposes of section 74(2)(b) of the Act, the prescribed particulars are—
(a)
the name of the incorporated association; and
(b)
the registration number of the incorporated association; and
(c)
the telephone number of the secretary (if available); and
(d)
the email address of the secretary (if available); and
(e)
the date of appointment of the secretary.
