6 Application for incorporation
For the purposes of section 6(3)(c) of the Act, the prescribed particulars are
(a)
the registered address of the proposed incorporated association; and
(b)
the postal address of the proposed incorporated association; and
(c)
the name, address, contact telephone number and email address of the applicant (if applicant is not the person nominated to be the first secretary of the proposed incorporated association); and
(d)
the number of members of the unincorporated association at the time of application; and
(e)
the estimated total revenue of the proposed incorporated association in its first financial year.
