61.06 Summons for examination
(1)
A summons under section 14 or 17 of the Act must be in Form 61F.
(2)
An application for issue of a summons under section 17 of the Act must be made by affidavit in Form 61G.
(3)
An affidavit under paragraph (2) may contain statements of fact based on information and belief if the grounds are set out.
