61.08 Forms of notice
A notice—
(a)
under section 6(4), 6(7), 7(4) or 18(2) of the Act must be in Form 61I;
(b)
under section 14(6) or 17(5) of the Act must be in Form 61J;
(c)
under section 6(6) or 8 of the Act must be in Form 61K.
