3.03 Application for statement of reasons
(1)
Rule 3.01(2) does not apply to an application to the Tribunal under section 47(1) of the Act.
(2)
An application to the Tribunal under section 47(1) of the Act must specify the following—
(a)
the name, address, telephone number and email address of the applicant;
(b)
the name, address, telephone number and email address of the decision‑maker;
(c)
the enabling enactment under which the application is made;
(d)
the decision;
(e)
the date of the decision.
