4 Information about appeals
For an appeal relating to the relevant patient’s order—
(a)
the fact, and date and time of hearing, of the appeal; and
(b)
a brief explanation of the nature of the appeal; and
(c)
a written statement of the decision on appeal identifying—
(i)
the date of the decision; and
(ii)
the decision made.
