an identifying name or number for the operation; and

the name and rank or position of the person granting the variation of authority; and

the date and time when the authority was varied; and

the provision of this chapter under which the variation was made; and

the period for which the variation has effect; and

if the variation is made under section 248—

the name of the applicant; and

whether the application was a formal variation application or an urgent variation application; and

a description of the variation having regard to the purposes mentioned in section 248(1) for which the application was made.
